During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced monitoring inspection was initiated on March 19, 2026 and completed on March 30, 2026 in response to a self-report received by the licensing office on March 9, 2026 relating to administration and special care provisions and emergencies. The inspector reviewed three children?s records on-site. This inspection included document review, tour of the facility, interviews, observations, and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to staff-to-children ratio requirements and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on April 6, 2026. A POC submitted after this date will not appear on the public website.

The required ratio for children ages 2 years old is one staff per eight children. One staff was observed supervising twelve children (youngest child 2 years old) from approximately 10:33 am-11:40 am.

Standard 8VAC20-780-80-B-2
Centers shall inform the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention. Child #1 received outside medical treatment for an injury sustained while in care at the center on September 29, 2026. It was noted on the injury report that EMS was called, and the Department was notified five months later.
