Inspector Notes

A monitoring inspection was initiated on 06/13/2023 and concluded on 06/13/2023. There were 9 children present, ranging in ages from 1 year to 4 years (22 pts), with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 provider/staff records were reviewed.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on observation, the provider failed to ensure that the diapering surface was nonabsorbent and washable.

Evidence:
1. The diapering changing pad on the changing table had several small visible rips on the pad and were nonabsorbent and washable.
Plan of Correction: I am planning on changing it out tomorrow.
