A subsidy health and safety (SHSI) inspection was initiated on 12/18/2023 and concluded on 12/18/2023. There were 9 children present with 2 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance with subsidy regulations and violations were documented on the violation notice issued to the facility.

Based on observation and record review, the vendor failed to maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.
Evidence:
1. The vendor had a sign in/out attendance sheet available at the front door. There were 9 children in attendance at 8:50 am but only 5 children were signed in on the attendance sheet.
2. The vendor confirmed there was only 5 children signed in and they did not get an opportunity to sign the rest of the children in yet.
