Inspector Notes
A renewal inspection was initiated on 12/18/2023 and concluded on 12/19/2023. There were 9 children present, ranging in ages from 23 months to 5 years, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 4 provider, staff and household records were reviewed.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.
Violations
2Evidence:
1. The record of Staff 2 (DOH 01/29/2016) did not contain documentation of a repeated central registry check. The central registry documentation on in the file was dated 06/28/2018.
2. The provider confirmed that she could not find the results of the central registry check but the employee did go to an appointment.
Evidence:
1. A tube of Destin diaper ointment was located in the drawer without a child's name on it.
2. A tube of Vitamin A & D ointment was located in with the diaper ointments and it was not labeled with a child's name.
3. The provider verified that the ointments did not have a child's name on them.
