During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 08/28/2025 and completed on 09/08/2025. The on-site inspection began at 12:20 PM and ended at 3:00 PM. The inspector reviewed compliance in the areas listed above. There were 11 children present and 2 caregivers at a total of 20 points. The inspector reviewed paperwork to be used once the license is approved. This inspection included: document review, tour of the facility and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/15/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-510-C
Occupied cots or rest mats shall be at least 12 inches from each other.

All nine cots did not have the required 12 inches from each other when occupied.

Standard 8VAC20-800-600-B
Infants and toddlers shall spend no more than 30 minutes of consecutive time during waking hours, with the exception of mealtimes, confined in a crib, play pen, high chair or other confining piece of equipment. The intervening time period between confinements shall be at least one hour.

Child # 1 was sitting in a swing for one hour and ten minutes. The infant was removed after 20 minutes to be swaddled and then returned to the swing.

To assist in preventing choking, hard food, round, small, thick and sticky, or smooth and slippery shall not be served to children under four years of age, unless the food is prepared before being served in a manner that will reduce the risk of choking,
