Inspector Notes
An allegation was received that the provider was operating a second daycare in a previous home. Based on interview statements, the allegation is unfounded. Non-related violations were found during the course of the inspection. A follow up consultation visit was conducted to ensure all children's records are complete and in order. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations
14Evidence:
Child #1 does not have a comb or toothbrush at the Family Day Home.
Evidence:
Child #1 sleeps upstairs and has access to the bathroom where personal items and cleaners were not locked or put out of reach of children.
Evidence:
A kitchen knife and scissors were in an unlocked drawer in the basement sink area.
Evidence:
Outlets in the large play room (13) as well as in the upstairs bedroom (12) were not covered.

The hall bathroom was missing paper towels.

The provider only had several games available. No books, art materials and additional games were accessible to children.

No written attendance was available to verify which children have been attending the previous week.
Evidence:
Observed child #6 was sleeping in a mesh sided crib upon the Licensing Inspector's arrival.
Evidence:
Child #6 was observed eating a hard lollipop as well as gagging on an orange peel.
Evidence:
Child #1(date of attendance February 24, 2022) and Child #2 (date of attendance unknown by the provider) records could not be found during the Licensing visit. Missing information includes a record of enrollment (parent/guardian information, doctor information, emergency contact information, proof of identity and emergency authorization), physical and immunization records as well Information for Parents regarding the home's policies and procedures.
Evidence:
Child #3 was missing an emergency contact.
Child #6 who is under two years of age was missing an updated immunization record (last updated 3/17/2021).
Child #7-Child #11 were missing a record of enrollment which includes parent/guardian information, doctor information, emergency contact information, proof of identity and emergency authorization.
Observed Child #4 and #6 eating fruit and neither washed their hands before eating snack.
Child #3,4,5,7,8,9,10 and 11 were missing all or parts of a completed Information for Parents form (including the provision of emergency relocation, medication policy or the liability declaration)
Evidence:
Child #6 was observed eating a hard piece of candy that he picked up off the floor.
