Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts and interviews.

A monitoring inspection was initiated on July 1, 2022 and concluded on 7/11/2022 regarding 3 self-reported incidents involving several staff and children at the center. The director contacted the licensing inspector by telephone to report the incidents and a virtual inspection was conducted. The inspector reviewed compliance in the areas of administration, staffing and supervision, programming.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on interviews with the Owner / Director, the licensee did not ensure that staff do not engage in forbidden actions or threats thereof to include physical punishment, striking a child, roughly handling or shaking a child, restricting movement through binding or tying, forcing a child to assume an uncomfortable position, or exercise as punishment.

Evidence:
The Owner / Director self-reported three incidents of staff engaging in forbidden actions on 7/1/2022.
The owner stated that a counselor (adult #4), who visits the center weekly to work with a child in care, reported observing the following incidents on 7/1/2022:
Staff #1 "pulled the chair out from under" an almost three year old child (child #1) causing him to fall to the ground.
Staff #2 and Staff #3 were said to have "Plucked" (Child #2, 2 years old) in the chest" and "tapped a child's hand" (child #3, unidentified male child from 2 year old class) while on the playground".
The Owner / Director was able to view video footage confirming the incident with staff #1 and child #1.
Plan of Correction: The owner removed all three involved staff from the center and contacted licensing and child protective services to report the incidents on the same day of the events.
