Inspector Notes
A monitoring inspection was initiated and concluded on 4/18/2023. There were 29 children, ages 16 months to 4 years old, present with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed. The inspection started at 10:35 am and concluded at 2:10 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
2Evidence:
There was a broken tricycle on the fenced play yard where toddlers, two year old children and preschool children ages 3 - 4 years old were playing.
The tricycle had the back portion of the hard plastic seat broken off and lying on the ground. The edges of the broken plastic were sharp. Staff were not aware where the broken plastic had come from. Upon looking around, the inspector discovered that the broken plastic was missing from a tricycle that a child was riding at that time. The remaining portion of the seat also had sharp edges.
The program director will remind staff to inspect the toys and equipment each time they take the children outdoors and remove any items that are in disrepair.
Evidence:
The following medications were accompanied by written authorization to administer for more than 10 days, signed by the parent, but not authorized or signed by a physician.
1. Excedrin Migraine pills (accompanied by a non-prescription topical authorization form)authorizing for the school year, signed only by the parent.
2. Children's Allegra accompanied by a medication authorization form signed only by the parent on 3/24/2023.
