A subsidy health and safety (SHSI) inspection was completed on 6/4/2023 A licensing inspection was also conducted on this date. Both inspections combined cover subsidy and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

** REPEAT VIOLATION**
Based on a review of records and interview, the licensee did not ensure that each staff annually completes the department's health and safety update course.
Evidence:
Five staff records were reviewed.
The record for staff #1 contained the most recently completed annual update to the department's health and safety update course was completed which was dated 4/30/2022. The program director confirmed that she was unable to demonstrate that a more recent update to the training had been completed.
