Inspector Notes
A monitoring inspection was initiated and concluded on 8/11/2023 regarding an incident that was self-reported to the agency on 8/9/2023 by the program. The inspector reviewed compliance in the areas of administration, staffing and supervision, programming. Observations were made with regard to the location of the reported incident and two staff were interviewed. The onsite portion of the inspection started on 8/11/2023 at 2:30 pm and concluded at 2:50 pm. Video footage and documentation were provided by the administration of the program and were reviewed by the licensing inspector outside of the facility, following the inspection. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
