Inspector Notes
A monitoring inspection was initiated and concluded on 9/25/2023. There were 34 children, ages 2 to five years old, present with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Children were observed playing in centers and with table top activities, eating lunch and settling onto cots for rest time. A total of 5 child records and 5 staff records were reviewed. The inspection started at 10:30 am and concluded at 12:30 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Repeat violations were found and are identified within this report.
Violations
2Evidence:
Five children's records were reviewed.
The record for child #2, whose enrollment date was documented as 12/1/2020, did not contain documentation, that the parent had confirmed, since enrollment, that the required information in the child's record is up to date.
The program director confirmed that there was not a review / update to the record documented.
Based on a review of records and interview, the licensee did not always keep all of the required records for each staff person.
Evidence:
Five staff records were reviewed.
The record for staff #1(employed 7/25/2023) contained documentation for only one of the two required references as to character and reputation as well as competency having been checked before employment.
The record for staff #1 did not contain the name, address, and telephone number of a person to be notified in an emergency which must be kept at the center.
A second reference will be obtained for staff #1 and placed in the record.
The program director will ensure that the name, address, and telephone number of a person to be notified in an emergency is documented in each staff record.
