Inspector Notes

A subsidy health and safety (SHSI) inspection was completed on 9/25/2023. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined violations with applicable standards or law. Violations, to include a Repeat violation, are identified on the violation notice.

Standard 8VAC20-790-600-D-1
Based on a review of records and interview, the licensee did not ensure that all staff who work directly with children shall have within 90 days of the date of employment, current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.
Evidence:
Five staff Records were reviewed.
The record for staff #5, whose date of hire was documented as 2/28/2023, did not contain documentation of certification in cardiopulmonary resuscitation (CPR).
The program Director confirmed that this documentation was not present in the record.
Plan of Correction: Staff #5 will complete certification in CPR appropriate to the age of the children she cares for, within 30 days and provide documentation of the certification for her record.
Standard 8VAC20-790-600-D-2
Based on a review of records and interview, the licensee did not ensure that all staff who work directly with children shall have within 90 days of the date of employment, current certification in first aid appropriate to the ages of children in care.

Five staff Records were reviewed.
The record for staff #5, whose date of hire was documented as 2/28/2023, did not contain documentation of certification in first aid.
The program Director confirmed that this documentation was not present in the record.
Plan of Correction: Staff #5 will complete certification in First Aid appropriate to the age of the children she cares for, within 30 days and provide documentation of the certification for her record.
Standard 8VAC20-790-600-F
Based on a review of records and interview, the licensee did not ensure that all staff who work directly with children annually attend at least 16 hours of training and staff development activities, to include the department's health and safety update course.
Evidence:
Five staff Records were reviewed.
The record for staff #3, who took the initial Virginia Preservice Training on 6/22/2023, did not contain documentation of completion of the departments health and safety update course.
The program Director confirmed that this documentation was not present in the record.
Plan of Correction: Staff #3 will complete the 3 hour annual update course within 30 days and submit documentation of the training for her file.
