Inspector Notes

A subsidy health and safety (SHSI) inspection was completed on 4/26/2024 from 1:20 pm - 4:40pm. At the time of entrance, 56 children, ages 1 year to 8 years old, were in care with 5 staff members present. The sample size consisted of 6 children?s records and 7 staff records

A licensing inspection was also conducted on this date. Both inspections combined cover subsidy and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined violations with applicable standards or law. Violations are identified on the violation notice.

Based on a review of records and interview, the licensee did not ensure that each staff annually completes the department's health and safety update course.

Evidence:
7 staff records were reviewed.
The record for staff #4 contained the last health and safety update training which was dated more than 12 months ago (4/10/2023).
The record for staff #5 contained the last health and safety update training which was dated more than 12 months ago (3/5/2023).
The assistant director confirmed that more recent training certificates were not available.
Plan of Correction: Staff #4 and Staff #5 will be asked to complete the update training and submit their certificates within 2 weeks.
