The licensing inspector initiated an unannounced inspection on 5/23/23 and concluded on 6/13/23 in response to a complaint received by the department on 5/16/22 for allegations regarding supervision and physical plant. Interviews and observations were conducted. The information gathered during the investigation does not support the allegation, so the complaint is determined to be "not valid."
