Inspector Notes
A subsidy health and safety (SHSI) inspection was conducted on 04/30/2024 from 10:40 AM to 12:20 PM. There were nine children present with two staff supervising ranging in age from thirteen months to three years. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance with subsidy regulations and violations were documented on the violation notice issued to the facility.

Based on record review and interview, the center failed to ensure staff obtained a current certification in CPR appropriate to the ages of children in care.
Evidence:
Staff 2's CPR certification expired 11/23.
Plan of Correction: The CPR certification for staff 2 will be updated as soon as a class can be scheduled. The provider will develop a system to ensure certifications do not expire in the future.
