Poisonous materials shall be stored in an area inaccessible to children.

Poisonous cleaning chemicals were on the floor in the kitchen and the children had access
to the chemicals.

Stationary outdoor playground equipment shall: Be placed at least six feet from the perimeter of other play structures or obstacles.

The record for Child #1, enrolled for eleven months, was missing the following required components: start date, previous child care, name, address and telephone number of one of the two designated persons to contact in case of an emergency if the parent cannot be reached.
The record for Child #3, enrolled for over two years, was missing the following required components: One of the two required designated persons to contact in case of an emergency if the parent cannot be reached did not have a complete address, allergies and intolerance to food, medication or any other substances, and actions to take in an emergency situation and annual updated emergency information.
The record for Child #4, enrolled for over two years, was missing the following required components: Chronic physical problem, pertinent developmental information and any special accommodations needed.

Medication, except for those prescriptions designated otherwise by a written Physician?s order, including refrigerated medication and medications for caregivers and household members, shall be kept in a locked place using a safe locking method that prevents access by children.

There were several containers of prescription medication sitting by the couch. One of the containers had pills that was not locked and accessible to the children in care.
