Inspector Notes

A monitoring inspection was initiated and concluded on 01/24/2024. The inspector arrived at 12:40 PM and departed at 2:40 PM. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 4 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the program director during the exit interview.

Standard 8VAC20-780-140-A
Based upon review of children's records and staff interview, the facility has not ensured that each child has a physical examination by or under the direction of a physician within 30 days of attending.
Evidence;
1. The record provided for child 1 who began attending on 12/4/2023 did not include the results of a physical examination by a physician.
2. Staff 1 confirmed that the record for child 1 did not include the results of a physical examination.
Plan of Correction: The facility responded with the following:
A physical examination will be obtained for this child as soon as possible. Children's records will be reviewed to ensure each child has a physical exam.

Based upon review of children's records and staff interview, the facility has not ensured that the child's physical examination was obtained within 12 months prior to attending.
Evidence:
1. The physical examination in the record for child 2, 3 yrs and 8 months old, who began attending on 6/26/2023, was dated 5/27/2020 which was not within the 12 months prior to attending.
2. Staff 1 confirmed that the physical for child 2 was not obtained within 12 months prior to attending.
Plan of Correction: The facility responded with the following:
The parent will be required to obtain a new physical examination for child 2. In the future, care will be taken to review every child's physical for information and timeliness.
