Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 04/19/2021 and concluded on 04/23/2021. The director was contacted by email to initiate the inspection. There were 40 children present and 9 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff records submitted by the facility to ensure documentation was complete. A video tour/observation was also conducted on 04/23/2021. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Based on record review and interview it was determined the center did not obtain a copy of the results of a sex offender registry check and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years prior to the first day of hire or service.
Evidence:
1. The record for staff #3(hire 3/29/21) did not contain documentation of a search of the child abuse and neglect registry and sex offender registry check for Florida. Staff #3 indicated on their sworn statement that they had lived in Florida within the last 5 years.
2. Staff #4 confirmed that there were no child abuse and neglect registry and sex offender registry check results from Florida for staff #3 available during this inspection.
Plan of Correction: Staff submitted the request to Florida on 4/21/21. Results will be forwarded to inspector once received.
