Inspector Notes
A joint investigation with local agency Child Protective Services was initiated from 10:30 am - 2:00 pm in conjunction with a monitoring inspection to investigate an allegation that was reported by the facility to the department regarding care, protection, and guidance. Interviews were conducted, documentation, video footage, staff records, and child records were reviewed. There was not a preponderance of evidence to support the reported allegation regarding care, protection, and guidance. Violations cited are not related to the self-reported allegation. Based on review of video footage it was determined that not all procedures for handwashing were followed. Evidence: After changing a child on the changing table, Staff A does not wash the child's hand. Staff A does not wash their hands after the diaper change, leaves on their soiled gloves and begins changing another child.
Plan of Correction: Staff will be retrained on handwashing procedures.

Based on review of video footage it was determined that not all procedures regarding diapering were followed. Evidence: In the toddler classroom Staff B is observed changing a child under three years old in the bathroom instead of on the changing table as required.
Plan of Correction: Staff will be retrained on diapering procedures.
