An unannounced monitoring inspection was initiated on 06/11/2025 and completed on 06/27/2025 with a local agency, in response to a self-report received by the licensing office on 06/05/2025 relating to Behavioral Guidance and a Forbidden Action. The inspector reviewed one children?s record and one staff record on-site.
