Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A complaint/monitoring inspection was initiated on 9/17/2021 and concluded on 10/6/2021. A complaint was received by the department regarding allegations in the areas of Staff to Child Ratios and Supervision.

The evidence gathered during the investigation did not support the (allegation(s)/self-report) of non-compliance with standards or law. Any violations not related to the (complaint(s)/self-report) but identified during the course of the investigation can be found on the violation notice.

Based on observation, areas/equipment of the center must be maintained in a clean safe and operable condition, including but not limited to chipped or peeling paint.
Evidence: observed chipped and peeling paint in the Junior Preschool Room.
Plan of Correction: Work orders are in place.
Standard 22VAC40-185-290-3
Based on observation, the facility did not ensure that electrical outlets were covered at all times.
Evidence: Observed one uncovered electrical outlet in the Junior Preschool Room and one uncovered outlet in the Toddler 2 classroom.
Plan of Correction: This has been done.
Standard 22VAC40-185-70-A
Based on review, the facility did not ensure that staff records contained the required minimum information.
Evidence: Reviewed 4 staff records and found that 4 did not have complete documentation of reference checks. Reference checks must contain dates of contact, names of person/firm contacted, results and signature of person making the call. None of the references had dates of contact and signature of the person making the call.
Plan of Correction: New form has been added.
