Inspector Notes

An unannounced Complaint Inspection was conducted today in response to a complaint that was received by the Virginia Department of Social Services, Licensing Division. Interviews were conducted with staff relating to allegations supervision, quantity and quality of staff, records. Based upon a preponderance of evidence, the information gathered during the investigation does not support the allegations, and the complaint is determined to be not valid. Please complete the columns for 'description of action to be taken' and 'date to be corrected' for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office within 10 business days. Based on observation, the facility did not ensure that areas and equipment of the center, inside and outside shall be maintained in a clean, safe and operable condition.
Evidence: observed in the senior school-age room a whole in the wall, along the far left wall.

Based on observation, the facility did not ensure that electrical outlets were covered with protective covers.
Evidence: observed the following uncovered electrical outlets:
1 - 1 uncovered outlet in the bathroom in the junior preschool room and 2 in the "circle time" area.
2 - 2 uncovered electrical outlets in the toddler bathroom.
