Inspector Notes
An unannounced renewal inspection was conducted on 7/12/2022 from 8:05am to 11:35am. There were 65 children (Infants to School-Age) directly supervised by 14 staff. The physical plant, 3 staff records, 5 children?s records, 10 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, individual activities, playing outside, eating snack. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Violations
8Evidence: observed that the step can in the 2s classroom was not operating properly and staff needed to use their hands to open.
Evidence: reviewed 3 staff records and found that Staff 3 did not have documentation of an updated TB screening or test.
Evidence: observed in the preschool classroom that the dramatic play table had a broken leg.
Evidence: observed in the Infant 2 classroom, a staff purse was stored low to the ground and accessible to children in the room. Identified through the staff member that owned the purse, that there were cosmetic items stored inside of the purse.
Evidence: observed on the "big" playground, the mulch was at approximately 2 inches in depth around the slide, stairs and the fall zones.
Evidence: observed that there was no shade provided on the "little" playground.
Evidence: observed in the Junior Preschool room, a child that was playing in the dramatic play area run over to the table and eat snack without washing their hands.
Evidence: Reviewed medication authorizations and found that Child 1 and Child 2 had expired medication authorization forms on site, and meds that had expired on site more than 14 days after their expiration dates.
