Inspector Notes

An unannounced complaint inspection was conducted on 6/30/2023 in response to a complaint that was received by the Virginia Department of Social Services, Licensing Division. Observations were made in each classroom and interviews were conducted with staff relating to allegations of equipment and materials. Based upon a preponderance of evidence, the information gathered during the investigation supports the allegations, and the complaint is determined to be valid. Please complete the columns for 'description of action to be taken' and 'date to be corrected' for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office within 10 business days. Based on observation, the facility did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence: observed in the Pre-K 2 classroom, a broken "krankle" toy with missing pieces, a "princess kitty" toy that had a large crack and a sharp edge.

Based on observation, the facility did not ensure that hazardous substances were kept in a locked place, using a safe locking method that prevents access by children.
Evidence: observed 3 bottles of shaving cream, marked "keep out of reach of children" on low shelves, accessible to children in the Preschool 1 classroom. Observed in the Pre-K2 classroom, 7 bottles of shaving cream, marked "keep out of reach of children" accessible to children on a low shelf.
