An unannounced monitoring inspection was initiated along with Licensing Inspector Keesha Minor on 10/26/23 in response to the center self-reporting an injury to a child that required outside medical care while under the supervision of the center. Interviews were conducted; equipment was inspected; two staff records and one child record were reviewed. Violations cited related to information submitted by the licensee. Repeat Violation
Based on a review of two staff files, the center did not obtain documentation of a tuberculosis (TB) screening for staff at the time of employment and prior to coming in contact with children.
Evidence: The record for Staff #1, with a start date of 07/01/23 and Staff 2, with a start date of 11/01/22, did not contain documentation of a tuberculosis screening at the time of inspection.

Evidence: On 10/18/23 at approximately 11:00 a.m. Child #1 fell from the middle point of the slide breaking their collarbone. The fall zone that had less than one inch of mulch, Six inches of mulch is required for the fall zone.
