Inspector Notes
An unannounced complaint investigation was conducted on 01/25/2024 from 9:14 a.m. ? 12:56 p.m. in response to a complaint received regarding allegations of failure to report injuries requiring outside medical treatment and toileting procedures. Based on a review of documentation and interviews conducted, the allegation regarding injuries requiring outside medical treatment and toileting procedures were determined to be valid. Violations
2Based on staff interviews and review of documentation, staff did not clean and change a child immediately when the child?s diaper became soiled.
Evidence: On the morning of 4/27/23, Child #1, two-years-old, was observed with feces in their hand, coming from the back of the diaper and on the carpet. An interview with staff revealed, they were instructed by administration to photograph the incident and send to the parent through the parent portal. Staff confirmed photographic documentation of the incident was obtained before they cleaned the child.
Teachers continue to be instructed to clean and change a child immediately when a child's diaper becomes soiled.
Evidence: On 4/27/23 at approximately 11:00 a.m. Child #1, two-years-old, sustained an injury to their fingers requiring outside medical attention. This incident was not reported by the facility to the Office of Child Care Health and Safety.
