During the inspection, the inspector reviewed. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of

Inspector Notes
An unannounced inspection was initiated on 04/14/25 and completed on 07/08/25 with a local agency, in response to a complaint received by the licensing office on 04/14/25 relating to allegations of administration and supervision. The inspector reviewed three children?s records and three staff records on-site on 04/21/25. This inspection included document review, tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 07/16/25. A POC submitted after this date will not appear on the public website.
Violations
2Staff became aware of a reportable offense on 04/11/25, and did not report it to the local department until 04/14/25.
On 04/11/25, the center was made aware of an allegation of suspected inappropriate behavior between Child #1, Child #2, and Child #3, while riding the center's bus.
On 04/14/25, the children were permitted to ride the bus together and interact without additional supervision. Child #1 became upset and started crying on the bus because Child #2 accused the child of telling on them. The center acknowledged additional supervision measures had not been put in place prior to allowing the children to ride together after the allegations were made.
