On 12/01/25 during morning snack, Child #1 (2-years-old) was sitting at a table with another child where an allergen was served. The child put her hands in the allergen and grabbed her cup. Child #1 touched the child?s cup and later touched his face. As a result, Child #1 suffered an allergic reaction, which included red, swollen eyes, lips, and itchiness. The child was administered emergency medications and was transported to a local emergency hospital.
Staff in the classroom were aware that Child #1 had a diagnosed food allergy, and no additional measures were taken to prevent cross contamination to ensure Child #1 was not near the allergen when it was served.
