Inspector Notes
An unannounced monitoring inspection was conducted today from 9:05am-10:20am. There were 6 children for a total of 16 points directly supervised by 2 staff. The home, 3 staff records, 3 children?s records, 1 household member?s record, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely inside and outside. There was an assortment of age appropriate materials for younger children but may want to have more for the older children. The home was clean and the Provider recently painted the children's area. The Inspector suggested that the Provider use a system that ensures all paperwork and documents are kept in one location so records are accessible and maintained for easy review. Areas of non-compliance are identified in the Violation Notice.

Violations
4Evidence:
1. Caregiver #3 has a CPS check dated March, 2018.
2. Clearances need to be resubmitted every 5 years.
3. Provider stated she did not realize it had been 5 years since last check.
Evidence:
1. Bathroom was checked and there were no paper towels. The Provider opened the closet next to bathroom and pulled out a package of facial tissues. Provider stated she though she had paper towels in the closet but would use these tissues.
Evidence:
1. The Provider has three cribs and two of the cribs have linen covers that are too big and do not tightly fit on the mattresses.
Evidence:
1. Provider has a radio but it only worked when plugged into the electrical outlet.
2. Provider placed batteries in the radio but it didn't work. She tried another set of batteries and those didn't work either.
