Standard 8VAC20-780-280-B
Hazardous substances must be in a locked place that prevents access by children.
1.In Classroom D there were containers of cleaner, disinfectants and air freshener in a bin on a shelf.
2. In Classroom E a container of bleach and water mixture was observed in an unlocked cabinet.
Containers were labeled ?keep out of reach of children? and at least one other statement, ?hazardous?, ?harmful? and ?flammable?.
Plan of Correction: That all hazards substances are locked up in the cabinet

Repeat Violation
Hand washing and toileting procedures were not followed by Staff #6.
1.Staff #6 was observed changing a diaper for child #6 and the child's hands and staff hands were not washed with soap and running water after the diaper change.
2.Staff #6 was observed serving snack to Child #6 and the staff hands were not washed with soap and water before serving snack.
3. Child #6's hands were not washed with soap and water or a disposable wipe before eating snack.
Plan of Correction: Talked to staff #6 about sanitizing and she did a training on diapering

Repeat Violation
The diapering surface shall be cleaned with soap and at least room temperature water and sanitized after each use. Diapering was observed in Room A, Staff #6 did not wash the diaper pad or sanitize it after changing a diaper.
Plan of Correction: Teacher had a training on diapering. Talked to #6 about sanitizing the diaper pad.

Repeat Violation
The center shall maintain a written record of children's serious and minor injuries in which entries are made on the day of occurrence. Five injury reports were reviewed and 2 did not include the future action to prevent recurrence of the injury and documentation on how parents were notified.
