Inspector Notes

A monitoring inspection was initiated on 7/14/2022 and concluded on 7/14/2022. There were a total of 9 children, including 1 of the provider's (24 points) observed with 2 caregivers present. The children were engaged in the following: toy play, morning snack, gross motor play, story time. A total of 3 child records and 2 caregiver/household member records were reviewed. Reviewed fire drills and emergency supplies. No medications. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on a review of caregiver records, the provider did not obtain for each caregiver a current Report of Tuberculosis Screening form, every two years from the date of the first screening.
Evidence: The following caregiver TB screenings have not been updated;
Caregiver #1, TB screening dated 2/26/2020
Caregiver #2, TB screening dated 3/17/2020
Plan of Correction: Caregivers will get their TB screening

Based on a review of children records, Before the child's first day of attendance, parents have not been provided in writing whether or not the there is liability insurance.
Evidence: Child #1, date of first attendance, 3/22/2022 record did not have documentation that the parent had received in writing the information about liability insurance.
Plan of Correction: Parents will sign the liability form and provider will file in the child's record.
