Inspector Notes

A monitoring inspection was initiated on 1/12/2023 and concluded on 1/12/2023. There were a total of 5 children ( 12 points) and 1 child of the providers observed with 2 caregivers present. The children were engaged in the following: card identification, story about vegetables, cutting and eating vegetables and gross motor play. The home, 3 child records, 2 caregiver/household member records, evacuation drills, and emergency supplies were inspected. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on a review of caregiver records and provider interview, caregiver has not completed a sworn statement prior to first day of employment.
Evidence: Caregiver #1 does not have documentation of a sworn statement. The caregiver was caring for children this date. The same caregiver was also caring for children during the last inspection on 7/14/2022
Plan of Correction: The caregiver will complete the sworn statement and file in his record.
Standard 8VAC20-770-60-C-2
Based on a review of caregiver records and provider interview, caregiver has not obtained a central registry finding within 30 days of employment.
Evidence: Caregiver #1 does not have documentation of a central registry finding. The caregiver was caring for children this date. The same caregiver was also caring for children during the last inspection on 7/14/2022
Plan of Correction: The caregiver will complete the central registry search and submit immediately. The results will be kept in the caretakers file.
