Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A subsidy health and safety inspection was initiated on May 27, 2021 and concluded on June 1, 2021. The provider was contacted by telephone to initiate the inspection. There was one child present and one caregiver. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection. The information gathered during the inspection determined non-compliance(s) with subsidy regulations, and violations were documented on the violation notice issued to the facility.

Based on review and interview, vendor did not ensure the department's annual health and safety update training is completed within a year of completing the department's preservice training.

Evidence: Provider completed the department's preservice training on 2/12/2020 and the health/safety update training has not been completed. Provider stated she was not aware the training needed to be done.
Plan of Correction: Provider will complete the training.
