Inspector Notes

An unannounced renewal inspection was conducted on August 17, 2023 from approximately 10:15 am to 11:30 am. Provider was present with five children. Children were observed playing freely in the childcare area. Five child, two caregiver and one household member records were reviewed. Violations found are noted on the violation notice.

Based on observation, provider did not ensure documents required by the Superintendent are posted in a conspicuous place on the licensed premises.

Evidence: The most recent inspection results dated 4/25/2023 were not posted.
Plan of Correction: Today's inspection notices will be posted.
Standard 8VAC20-800-120-B
Based on review and interview, provider did not ensure an assistant/substitute record had documentation of required information.

Evidence: The record for caregiver #2 did not have documentation of date of employment and job title. Provider stated the individual began working the week of 7/24/2023.
Plan of Correction: Provider will locate the information and add it to caregiver record.
Standard 8VAC20-800-230-A
Based on review and interview, provider did not ensure there is written documentation of each caregiver's orientation training.

Evidence: The record for caregiver #2 did not have documentation of orientation training although provider stated it was completed.
Plan of Correction: Provider will locate orientation training document and place in caregiver's record.

Repeat Violation
Based on review, provider did not ensure 4 of 5 child records have documentation of required information.

Evidence: The following was found during review of child records:
1. The records for child #1 and child #2 did not have parents' employment addresses and employment phone numbers.
2. The records for child #4 and child #5 did not have father's home information (address and phone number), parents' employment information (place, address and phone number) and emergency contact addresses.
Plan of Correction: Provider will get information from parents and add to each child's record.
