Inspector Notes

A renewal inspection was initiated on 7/13/2022 and concluded on 7/13/2022. There were a total of 1 child (2 points) observed with 1 caregiver present. The child arrived at the end of this inspection. A total of 1 new child records and 2 caregiver/household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-800-100-A
Based on a review of records and provider interview, the provider did not obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.
Evidence: Child #1, date of first attendance 3/21/2022 does not have documentation of a physical exam. The provider stated that she did not obtain a copy of the physical exam.
Plan of Correction: The provider will ask the parent to get a copy of the child's physical exam.
Standard 8VAC20-800-180-A
Based on a review of records and provider interview, the provider did not obtain for each household member a current report of tuberculosis screening every two years from the date of the last screening.
Evidence: Household #1 does not have a current TB screening on file, the one on file is dated 5/1/2018.
Plan of Correction: The Household member will get the TB screening and put the results in the record.
