Inspector Notes
A monitoring inspection was initiated on 1/10/20213 and concluded on 1/10/2023. There were a total of 3 children ( 9 points) observed with 2 caregivers present. The children were engaged in the following: toy play. The home, 4 child records, 3 caregiver/household member records, evacuation drills, and emergency supplies were inspected. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations
3Evidence: The provider has not equipped the Family Day Home with a carbon monoxide detector.
Evidence: Child #1, date of enrollment 8/29/2022 the record does not have documentation of the medical insurance name and policy number.
Child #2, date of enrollment 1/10/2022 the record does not have documentation of the child's address, the mothers address, the moms employer name, 1 emergency contact name, address and phone number.
Evidence: The diapering surface was observed with a white cloth and a colored cotton covering over the changing pad. The changing surface was not nonabsorbent.
