The most current violation notice, dated 7/9/2025 was not posted. The posted violation notice was dated 7/7/2023.

The center is required to obtain a completed sworn statement prior to the employee's first day of employment.
There is no sworn statement for caregiver #1, who is currently caring for children.

Caregiver #1 was observed at the home at 8:45 am alone with one child until 9:05 am. The caregiver could not speak English. The provider was not at home during the inspection. Caregiver #2 arrived to the home at 9:05 am
Plan of Correction: The caregiver will always be with other caregiver to assigned job responsibilities.

Poisonous cleaning chemicals were found in a unlock cabinet in the unlocked bathroom and also on the feeding table where children were present eating breakfast.
Plan of Correction: I will lock the poisonous material.

There were no attendance records at 8:45 am on the date of inspection. The provider was not at the home, caregiver #1 was caring for one child alone. At 9:05 am the assistant and 4 other children arrived, there was still no attendance taken.

An up to date record for each child enrolled shall be maintained by the provider.
There was no record for child #5, who was in care during the inspection and had been in care for 2 weeks.
Plan of Correction: The parent completed the forms.

Children's records are required to contain name, address and phone number of two designated persons to contact in case of emergency.
The record for Child #3 was missing a different second name, address and phone number of a person to contact in case of emergency. The child was present during the inspection and had been in care for over 2 years.
Plan of Correction: I will ask the parent for information.

The emergency evacuation procedures shall be practiced monthly with all caregivers and children in care during all shifts that children are in care.
There was no documentation of emergency evacuation drills.
Plan of Correction: I will put the record of emergency drills on the parent wall.
