Inspector Notes
A renewal inspection was conducted today, January 9, 2023. The inspector was on site from approximately 2:15 pm until 5:25 pm. The director arrived shortly after the Inspector and assisted in the inspection. There were 32 children present, ranging in ages from six months to five years, with five staff supervising. Upon the Inspector?s arrival, children were napping. Later children were observed playing with toys, watching videos and playing cards. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and five staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. If you have questions regarding this inspection, please contact the licensing inspector.

Violations
10Evidence:
1. The record for Child #2 (first day of attending center 10/03/2022) contained an immunization record dated 06/04/2022. Child #2 is under two years of age. Updated immunization records were required to be obtained.
2. The record for Child #4 (first day of attending center 05/16/2022) contained an immunization record dated 05/12/2022. Child #4 is under two years of age. Updated immunization records were required to be obtained.
3. A member of management confirmed the information during the inspection.

The director stated she could not locate the current fire inspection report
Evidence:
1. The director stated that medication was not administer at the center.
2. The parent handbook stated ?No Medication will be given at the daycare by staff. At this time, no medication will be administered by the staff?.
3. The staff handbook states, ?Medication will be administered by the director only?.
Evidence:
In the two year old classroom, eight children were observed napping on cots. Four of the cots were less than 12 inches away from each other
Evidence:
1. The record of staff #1, who was observed alone with a group of eight school age children, did not contain documentation of current CPR. The director stated she did not have documentation of the staff completing CPR certification.
2. The record of staff #4, who was identified as the van driver and alone on the van with school age children, had documentation of CPR certification dated 10/11/2022 however the training did not have an in-person competency demonstration.
3. The record of staff #5, who was observed alone with a group of 4 infants, had documentation of CPR certification dated 03/27/2022 however the training did not have an in-person competency demonstration.
4. The record of staff #6, who was observed alone with a group of 8 two year olds, had documentation of CPR certification dated 09/01/2022 however the training did not have an in-person competency demonstration.
5. The record of staff #7, who was observed alone with a group of 8 preschool children, had documentation of CPR certification dated 01/17/2022 however the training did not have an in-person competency demonstration.
6. The record of staff #8, who was observed alone with a group of 5 toddler aged children, did not have documentation of CPR certification. The director stated she did not have documentation of staff #8 completing CPR certification.
Evidence
1. The record of staff #1, who was observed alone with a group of eight school age children, did not contain documentation of current first aid certification. The director stated she did not have documentation of the staff completing first aid certification.
2. The record of staff #8, who was observed alone with a group of 5 toddler aged children, did not have documentation of first aid certification. The director stated she did not have documentation of staff #8 completing first certification.
Evidence:
Three injury reports reviewed did not have documentation of the time when parents were notified of the injury.
Evidence:
Child #2, 17 months in age, was observed sitting in the bucket seat of the feeding table. The protective belt was not fastened.
Evidence:
Two prepared bottles were observed in the refrigerator. The two bottles were not labeled with a name or date. Staff #5 identified the two bottles as belonging to child #4.
Evidence:
The record of staff #1, staff #5, staff #6, staff #7 and staff #8 (all identified as program leaders) did not contain to demonstrate that the individual possesses the education, certification, and experience required by the job position.
