Inspector Notes

An unannounced, on-site code compliance inspection was conducted on 12/11/2024. The on-site inspection began at 10:50am and ended at 12:25pm. The inspector reviewed compliance in the areas listed above. There were 18 children present with 3 staff. The inspector reviewed 4 child records and 4 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance(s) with applicable code sections, and violations are documented on the violation notice issued to the program.

The center did not have documentation of a person trained to perform daily health screenings.

Staff #1, #2, #3, and #4 did not have documentation of child abuse and neglect training.
Plan of Correction: The Child Abuse and Neglect training for staff #4, dated 5/18/2023, was emailed to the LI after the time of inspection. We will schedule training for the remaining staff.
