Inspector Notes
An unannounced monitoring inspection was conducted on 1/26/2023 at 12:20pm. There were 110 (Infants to 5yrs) directly supervised by 21 staff. The physical plant, 5 staff records, 5 children?s records, children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed eating lunch, and preparing for and taking naps. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Violations
4Evidence: observed peeling paint and a hole in the wall in Pre-K 2 classroom.
Evidence: observed int he toddler classroom, a cabinet with Woolite carpet cleaner and rubbing alcohol in an unlocked cabinet.
Evidence: observed a container of COLD EASE an NSAID in an unlocked cabinet in the Toddler classroom.
Evidence: reviewed 5 children's records and found that Child 1 and 3 did not have documentation of annual parental updates.
