Inspector Notes

An unannounced monitoring inspection was conducted on 1/26/2023 at 12:20pm. There were 110 (Infants to 5yrs) directly supervised by 21 staff. The physical plant, 5 staff records, 5 children?s records, children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed eating lunch, and preparing for and taking naps. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Based on observation, the facility did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence: observed peeling paint and a hole in the wall in Pre-K 2 classroom.
Plan of Correction: Maintenance was contacted and will be corrected by the end of February.

Based on observation, the facility did not have hazardous substances kept in a locked place using a safe locking method that prevents access by children.
Evidence: observed int he toddler classroom, a cabinet with Woolite carpet cleaner and rubbing alcohol in an unlocked cabinet.
Plan of Correction: Corrected at the time of the inspection.

Based on observation, the facility did not ensure that medication was kept in a locked place using a safe locking method that prevents access by children.
Evidence: observed a container of COLD EASE an NSAID in an unlocked cabinet in the Toddler classroom.

Based on review, the facility did not ensure that children's records were maintained with the required annual parental updates.
Evidence: reviewed 5 children's records and found that Child 1 and 3 did not have documentation of annual parental updates.
