Inspector Notes
An unannounced monitoring inspection was conducted on 7/24/2023 from 12:56pm to 2:30pm. There were 108 (infants to 5yrs) directly supervised by 15 staff. The physical plant, 4 staff records, 5 children?s records, 7 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed preparing for naps, napping, or interacting quietly with staff while other children napped. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Violations
4Evidence: reviewed 5 children's records and found that Child 1 and Child 3 did not have signatures on their immunization records.
Evidence: observed a powerstrip (surge protector) in PreK 1 (rm 8) that was accessible to children, and several of the outlets were not covered or locked.
Evidence: observed that there was less than 12 inches between occupied cots in the Toddler, Room 4.
Based on review, the facility did not ensure that children's records were maintained with the required minimum information.
Evidence: reviewed 5 children's records and found that Child 1 did not have documentation of addresses for both emergency contacts.
