Inspector Notes

An unannounced monitoring inspection was conducted on 7/24/2023 from 12:56pm to 2:30pm. There were 108 (infants to 5yrs) directly supervised by 15 staff. The physical plant, 4 staff records, 5 children?s records, 7 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed preparing for naps, napping, or interacting quietly with staff while other children napped. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Based on review, the facility did not ensure that documentation of dates of immunizations received was signed by a physician, their designee, or an official of a local health department.

Based on observation, the facility did not ensure that electrical outlets had protective covers.
Evidence: observed a powerstrip (surge protector) in PreK 1 (rm 8) that was accessible to children, and several of the outlets were not covered or locked.
