Violations
2Evidence: The bottles for three of the infants (Child #1, Child #11, and Child #12) in the infant room did not have the date on the labels. The Assistant Director acknowledged that the bottles weren?t dated as required.
Evidence: The written agreements between the parents and the center in ten children?s records (Child #1 through Child #10) did not include a statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life threatening diseases which must be reported immediately. The Assistant Director acknowledged that the statement was not included in their parental agreements.
