Inspector Notes

A renewal inspection was conducted on July 12, 2024, from approximately 9:30 A.M. to 1:18 P.M., 99 children were present, ranging in ages from four months to five years with 19 staff supervising. The inspector reviewed compliance in the areas of administration, staff qualifications and training, physical plant, staffing and supervision, programs, special care and emergencies, special services, and background checks.

During the inspection, children were observed participating in teacher-directed and self-directed activities. Staff were observed having positive interactions with the children. The center is equipped with toys and supplies and items were available to the children. Ten children?s records and five staff records were reviewed. Background checks for two corporate officers were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or laws and violations were documented on the violation notice issued to the program. Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it within 5 business days. Please specify how the deficient practice will be or has been corrected.

Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard 8VAC20-780-570-E
Based on observation of four infants? bottles and interview, the center did not ensure that three infants? bottles were dated as required.

Evidence: The bottles for three of the infants (Child #1, Child #11, and Child #12) in the infant room did not have the date on the labels. The Assistant Director acknowledged that the bottles weren?t dated as required.
Plan of Correction: Parents are informed and reminded to label all infant bottles with names and date and teacher will double check.
Standard 8VAC20-780-90-A
Based on a review of ten children?s records and interview, the center did not include required information in their written agreements with parents.

Evidence: The written agreements between the parents and the center in ten children?s records (Child #1 through Child #10) did not include a statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life threatening diseases which must be reported immediately. The Assistant Director acknowledged that the statement was not included in their parental agreements.
Plan of Correction: All files going forward will include a written statement that states that the parent will inform the center 24 hours on next business day after his child or any member has developed and reportable communicable disease.
