During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated and completed on 6/26/2025 in response to a complaint received by the licensing office on 6/24/2025 relating to supervision. The inspector interviewed three staff members on-site. This inspection included document review (i.e. Fire Safety policy and procedures, a fire drill log, and an attendance record), tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 7/14/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance. Staff #4 did not ensure the care, protection, and guidance of Child #1 on 6/11/2025. The fire alarm went off at 2:10 p.m. and the children were evacuated outside. Child #1, age 4, was left behind, napping in her classroom. Child #1 was alone and unsupervised for approximately 1-2 minutes before Staff #2 realized she was still inside.

Children under 10 years of age always shall be within actual sight and sound supervision of staff. On 6/11/2025, Child #1, age 4 years was out of sight and sound supervision of staff from approximately 2:10 p.m. to 2:12 p.m. when the rest of the children in her classroom were evacuated after the fire alarm went off.

The operational responsibilities of the licensee shall include ensuring that the center's services are maintained in compliance with the center's own policies and procedures that are required by these standards. Staff #4 did not follow the Fire Safety Policy regarding the evacuation procedure. When the fire alarm went off on 6/11/25 at 2:10 p.m., Staff #4 did not ?use the attendance sheet or the tablet to verify that the head count was the same as the count documented on the attendance sheet.? Child #1, age 4, was left in the classroom alone and unsupervised for approximately 1-2 minutes.
