During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced monitoring inspection was initiated and completed on 8/15/2025 in response to a self-report received by the licensing office on 7/31/25 relating to supervision. The inspector reviewed one child?s record and two staff records on-site. This inspection included document review (i.e. Incident reports, policies and procedures, name to face trackers, attendance records), tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 8/27/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-A
(Repeat/Systemic Violation) When staff are supervising children, they shall always ensure their care, protection, and guidance. Staff #2 and Staff #3 did not ensure the care, protection, and guidance of Child #1 on 7/31/2025. Child #1 was left alone on the playground when the rest of the children in his classroom returned to their classroom at 9:42 a.m. Child #1 remained outside, alone, for approximately 22 minutes until a second classroom came outside at 10:04 a.m. and teachers realized he was still out there.

(Repeat/Systemic Violation) Children under 10 years of age always shall be within actual sight and sound supervision of staff. Child #1, age 3 years was left outside, out of sight and sound supervision of Staff #2 and Staff #3 for approximately 22 minutes from 9:42 a.m. to 10:04 a.m. on 7/31/25. Child #1 was left on the playground alone when the rest of the children in his classroom returned to their classroom at 9:42 a.m. until a second class came outside at 10:04 a.m.

(Repeat/Systemic Violation) The operational responsibilities of the licensee shall include ensuring that the center's services are maintained in compliance with the center's own policies and procedures that are required by these standards. Staff #2 and Staff #3 did not follow the ?Supervision Policy? on 7/31/25 before they returned to their classroom from the playground with the children. The policy states that ?Accurate attendance must be kept at all times by teachers. All forms of attendance must accurately reflect the children who are present.? On 7/31/25, Child #1, age 3, was left alone on the playground when the rest of the children in his classroom returned to their classroom at 9:42 a.m. Child #1 remained outside, alone, until a second classroom came outside at 10:04 a.m. and teachers realized he was still out there.
