Inspector Notes
An unannounced monitoring inspection was initiated and completed on 12/02/2025, as a follow up to an Intensive Plan of Correction and a mandated monitoring inspection. The inspector reviewed 10 children?s records and five staff records on-site. This inspection included document review (i.e. policies and procedures, attendance records, Injury logs, emergency drill logs, medication authorizations, fire inspection, and health inspection), tour of the facility, interviews, and observations.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
