Inspector Notes
A monitoring inspection was conducted on October 26, 2023, from approximately 9:15 AM to 11:40 AM. There were 24 children present in the center, ranging in ages from infant to preschool, with eight staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and six staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures.

Evidence:
1. In room #1, one container of Clorox wipes was on top of cubbies. One container of Clorox All Purpose cleaner was unlocked under the diaper changing table.
2. In room #2, one container of Clorox wipes, one container of Lysol, one container of Febreze, and one container of Awesome disinfectant cleaner was unlocked under the diaper changing table.
Evidence:
1. In room #3 and room #6, the electrical outlets were not covered. In room #3 children in care are infants and in room #6 children in care are preschool age.
Evidence:
The records for staff #3 (employed 10/24/22) and staff #6 (employed 10/23/23) did not contain documentation of two or more references before employment.
