Inspector Notes

An unannounced monitoring visit was conducted today. Four children were in care with one caregiver for eight points which is within ratios for supervision. Observed children transitioning from lunch (pasta with meat sauce, milk and strawberries), preparation for rest time, diapering and hand washing, free play and reading books. Reviewed one child record and provider training, emergency preparation and attendance. Home has dedicated space on the lower level of the the townhouse with 2 rooms for care and an updated outside play area. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation and provider statement, no written authorization was available for 2 medications.
Evidence"
Child #1 has 2 emergency medications on-site and no written authorization form completed by the parent or guardian.
Plan of Correction: The provider will give the parent the 2 authorization forms to be completed by the parent and the physician.
Standard 8VAC20-800-710
Based on observation, a prescription medication is not in the original container with the pharmacy label identifying the child, dosage, route of administration and frequency.
Evidence:
The medication for Child #1 was loose in a ziplock bag and did not have the corresponding pharmacy label.
Plan of Correction: The provider will request the parent bring in the missing box or label from the pharmacy with all required information.
