Inspector Notes

A new on-site renewal inspection was initiated and completed on 08/01/2024, . The on-site inspection due to a change in the provider's business entity. The inspection began at 10am and ended at 11:50am. The inspector reviewed compliance in the areas listed above. There were 6 children present and 2 caregivers; the point total was20 points. The inspector reviewed 5 children?s records and 3 staff records on-site. This inspection included document review, tour of the home, observations and staff interviews. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on child record review, documentation that two children were adequately immunized was not obtained before their first day of attendance.
Evidence:
Child # 1 and #3 began attending the family day home on 07/29/2024. Documentation of immunizations for both children has not been obtained.
Plan of Correction: I will require both parents to provide the required documentation.
